Thank you for using the ePHC App ("App"), developed by ZemSys Technologies ("we," "us," or "our").

when you use the App to track attendance at your Primary Health Center (PHC). By using the App,
you agree to the practices described in this policy.
1. Information We Collect
We collect the following information to provide and improve the App’s services:
- Personal Information:
- Names: Your full name as provided during registration.
- Email: Your email address for authentication and communication.
- Phone Number: Your phone number for account verification and support.
- Cadre: Your professional role or designation (e.g., nurse, community health extension worker).
- PHC of Assignment: The name and geolocation (latitude and longitude) of your assigned
Primary Health Center, which you provide via this link.
- Profile Photo: An optional photo you may upload to personalize your profile.
- Device ID: A unique identifier of your device, captured during your first login and stored for authentication purposes.
- Usage Data:
- Clock-In/Out Data: Time and geolocation (within 100m of your PHC) when you clock in or out,
including offline records synced when a network is available.
- Attendance History: Records of your clock-in/out activities stored in the App’s dashboard.
- Monthly Reports: Data submitted via the monthly report form, which may include health facility/LGA performance
and attendance summaries.
- Technical Data:
- Device information (e.g., model, operating system) to ensure compatibility and security. This is NOT collected in
this current version of the App
2. How We Use Your Information
We use your information for the following purposes:
- To authenticate your identity and validate your clock-in/out activities using your email, password, and device ID.
- To track and record your attendance at your assigned PHC based on geolocation data.
- To provide offline clock-in/out functionality and sync data when a network is available.
- To generate attendance history, dashboards, and monthly reports for your use and submission to your PHC management.
- To improve the App’s functionality, troubleshoot issues, and enhance user experience.
- To communicate with you about updates, support, or account-related matters.
- To ensure compliance with health care and data protection regulations.
3. How We Share Your Information
We do not sell your personal information. We may share it in the following circumstances:
- With PHC Management: Attendance data and monthly reports may be shared with
your PHC administrators or health authorities for operational purposes.
- With Service Providers: We may engage third-party service providers (e.g., cloud storage
or analytics providers) to process data on our behalf, under strict confidentiality agreements.
- Legal Compliance: We may disclose information if required by law, court order, or to
protect our rights, safety, or property.
- Business Transfers: In the event of a merger, acquisition, or sale of assets, your
information may be transferred to the new entity, subject to this Privacy Policy.

If we become aware of such collection, we will delete the information promptly.

changes via the App or email. Continued use of the App after updates constitutes acceptance of the new policy.
